This form is to report an incident or challenging experience that occurred at an ISTA event, or in respect to an ISTA faculty or team member.

You may fill this form out for yourself or on behalf of another. The form may be anonymous, if you choose.

If you need assistance or support filling out this form, or are more comfortable reporting through a neutral third party hired by ISTA to support with the resolution of any grievances, rather than reporting through this channel which goes directly to the ISTA Feedback Stream, you may submit a report through our Assisted Feedback to ISTA portal which is hosted on Safe Mediation’s (“SM”) website at Safe Mediation

INFORMATION & CONFIDENTIALITY

ISTA's first step after receiving the report will be to contact the person who brought the issue forward to see if they want a conversation. This should happen within 7 days. Further action depends on the results of that conversation. If the report comes through Safe Mediation’s Assisted Feedback Channel for ISTA, then SM will contact the person who brought the issue forward. If the report is anonymous, ISTA will investigate the information provided and take appropriate action.

Your incident report will be received by members of the ISTA Feedback Stream and may be shared with members of the ISTA Accountability Stream if your report involves a member of ISTA faculty who is currently a Candidate in our Accountability Process (detailed info on our Accountability Process and the journeys of current Accountability Candidates can be found on our Accountability Process Page). Incident Reports sent directly to ISTA through this online form may be shared with our circle of Lead Faculty (only if that fits within the confidentiality criteria you designate on your form). That said, we will follow the protocols you indicate on your form submission as far as with whom you want your Incident Report shared and not shared.

ISTA's approach to receiving Incident Reports will involve taking your feedback to heart, creating an opportunity to hear the voice(s) of the person(s) that are named in your report, and intending to find a way to repair.
